JOUW PRIVACY BIJ BEHANDELING

Voor een goede behandeling is het noodzakelijk dat ik, als je behandelende therapeut, een dossier aanleg. Dit is ook een wettelijke plicht opgelegd door de WGBO. Je dossier bevat aantekeningen over je gezondheidstoestand en gegevens over de uitgevoerde behandelingen.
Ook worden in het dossier gegevens opgenomen die voor je behandeling noodzakelijk zijn en die ik indien nodig, na je expliciete toestemming, heb opgevraagd bij een andere zorgverlener – bijvoorbeeld bij de huisarts.

Ik doe mijn uiterste best om je privacy te waarborgen. Dit betekent onder meer dat ik:

  • zorgvuldig omga met je persoonlijke en medische gegevens,

  • ervoor zorg dat onbevoegden geen toegang hebben tot je gegevens

Als je behandelende therapeut heb ik als enige toegang tot de gegevens in je dossier.
Ik heb een wettelijke geheimhoudingsplicht (beroepsgeheim).
De gegevens uit je dossier kunnen ook nog voor de volgende doelen gebruikt worden:

  • Om andere zorgverleners te informeren, bijvoorbeeld als de therapie is afgerond of bij een verwijzing naar een andere behandelaar. Dit gebeurt alleen met je expliciete toestemming.

  • Voor het gebruik voor waarneming, tijdens mijn afwezigheid.

  • of voor het geanonimiseerde gebruik tijdens intercollegiale toetsing.

  • Een klein deel van de gegevens uit je dossier wordt gebruikt voor de financiële administratie, zodat ik of mijn administrateur een factuur kan opstellen.

Als ik vanwege een andere reden gebruik wil maken van je gegevens, dan zal ik je eerst informeren en expliciet om je toestemming vragen.
Deze gegevens in het cliëntendossier blijven zoals in de wet op de behandelovereenkomst wordt vereist 15 jaar bewaard.

YOUR PRIVACY DURING TREATMENT

For proper treatment, it is necessary that I, as your treating therapist, maintain a client file. This is also a legal requirement under the Medical Treatment Contracts Act (WGBO). Your file contains notes about your health condition and details of the treatments carried out.

The file may also include information that is necessary for your treatment and that I have obtained—if needed and only with your explicit consent—from another healthcare provider, such as your general practitioner.

I make every effort to safeguard your privacy. This means, among other things, that I:

•                 handle your personal and medical data with care,

•                 ensure that unauthorized persons do not have access to your data.

As your treating therapist, I am the only person who has access to the data in your file. I am bound by a legal duty of confidentiality (professional secrecy).

The data in your file may also be used for the following purposes:

•                 to inform other healthcare providers, for example when therapy has been completed or in the event of a referral to another practitioner. This will only take place with your explicit consent;

•                 for coverage/continuity of care during my absence;

•                 for anonymized use in peer review.

A small portion of the data from your file is used for financial administration, so that I or my administrator can prepare invoices.

If I wish to use your data for any other purpose, I will first inform you and explicitly request your consent.

The data in your client file will be retained for 15 years, as required by law under the treatment agreement regulations.
